To expedite the check-in process, please fill out the registration forms online prior to your appointment. A link to these forms will be texted and e-mailed to you.
What to Bring
You will need to bring the following on your first visit:
- Driver’s license or photo ID
- Dental insurance information and card (if applicable)
Please assist us by ensuring the following documentation is brought with you or sent to us by your dentist (by email or fax) prior to your first visit:
- The referral from your dentist
- X-rays from your dentist
What to Expect
Your initial consultation visit with Dr. Lam will include:
- An overview of your medical and dental history
- Oral cancer screening, including an extraoral head and neck exam
- Comprehensive periodontal and/or implant evaluation
- Review of your chief concern and answering any other questions you may have
- Treatment plan discussion
